PRESS RELEASE: Care Ring forms partnership to address social drivers of health and well-being

CHARLOTTE, NC – Care Ring was recently awarded a grant from the Kate B. Reynolds Charitable Trust in Winston-Salem, NC that will fund efforts to support health systems and community-based organizations as they work to improve health outcomes for Medicaid and uninsured populations by addressing social drivers of health and well-being.


The two-year, $440,000 award will enable Care Ring to undertake this work in partnership with fellow award winners and community-based organizations, the UCITY Family Zone and CrossRoads Corporation for Affordable Housing. “CrossRoads Corporation is grateful for the support from the Kate B. Reynolds Charitable Trust and we are excited to partner with Care Ring to focus on improving health outcomes for the residents of the Grier Heights community,” said Tiffany Capers, Executive Director of CrossRoads Corporation.

With a focus on social drivers of health and the social connectivity of neighborhoods, this project will help Care Ring and our community better understand how health systems can align with communities to drive local priorities and engage community leadership to increase health promotion and disease prevention.


“By combining and aligning our efforts we hope to refine and test evidence-based approaches that can be used to guide Medicaid transformation and other types of health reform efforts, in ways that actually improve population health outcomes over the life course for residents in low-income communities,” said Mark J. DeHaven, UNC Charlotte Dean W. Colvard Distinguished Professor of Community Health and Co-Director of the UCITY Family Zone.


Care Ring will lead the effort to build greater connection between existing health systems and community-based organizations.

“We know that social determinants of health, such as housing and transportation are the biggest predictor of long-term health outcomes,” said Don Jonas, Executive Director of Care Ring. “We’re thrilled to partner with CrossRoads Corporation and UCITY Family Zone to learn how healthcare systems can better address these critical issues and align with community-based organizations to help improve the health of our County’s most vulnerable residents.”
