®
Nurse-Family Partnership® (NFP) empowers moms living in poverty to successfully change their lives and the lives of their children through evidence-based nurse home visiting.
As an evidence-based primary prevention program, studies have shown the following consistent program outcomes: improved prenatal health, fewer subsequent pregnancies, increased maternal employment, increased intervals between births, improved school readiness and fewer childhood injuries.
To participate in NFP clients must:
· Be pregnant*
· Meet income requirements
· You qualify if you are eligible for Medicaid/WIC
· Live in Mecklenburg County
*must be less than 28 weeks pregnant if it's your first child
